I hereby agree with the following information:

• I understand that the counsellor is not a doctor, psychiatrist or psychologist but a Specialist Wellness counsellor or Holistic counsellor registered with the ASCHP.
• Session costs are not covered by medical aid and I am responsible for payment.
• The duration of a counselling session is 45 minutes to 1 hour.
• Missed time due to being late will not be accommodated due to other scheduled sessions unless otherwise stated.
• A session will be finalised and commence only once full payment is received.
• I will be liable for the full consultation fee if an appointment is not cancelled 24 hours before.
• Notes might be taken during the sessions. I understand that this information will be kept confidential.
• Counsellors cannot see a person under the age of 18.
• Counsellors do not issue formal reports.

Confidentiality agreement:

Confidentiality will be maintained between the counsellor and the client except for the following reasons:

• If my counsellor notes that I am a danger to myself or others.
• If there is any suspected child abuse, elder abuse or abuse of people with disabilities.
• If it is mandated by the law for legal proceedings.
